The Center for Inherited Blood Disorders (CIBD) is a mission-driven non-profit focused on delivering  high-quality, comprehensive, and family-centered care to improve the quality of life for children and adults with blood disorders.

Job Summary
Reporting to the Clinic Manager, the Care Manager coordinates the care and services of CIBD patients across the continuum of illness; promotes and monitors effective utilization of health care resources; and, assumes a leadership role with the multi-disciplinary care team to achieve optimal clinical and resource outcomes. As part of the Case Management Team, the Care Manager provides Team Center Care in a collaborative setting in accordance with the standards and regulations for California Children’s Services (CCS), Genetically Handicapped Persons Program (GHPP) and all other State and Federally sponsored programs.

Essential Duties
 
  • The Care Manager works with patients to understand their needs and their understanding of the plan of care and then coordinates their care with outside facilities including but not limited to home health agencies, third party payers, pharmacies, lab facilities.
  • The Care Manager works with internal (the physicians, nurses and authorization coordinator) and external (home health agencies, third party payers, pharmacies, lab facilities and other outside providers) partners to ensure proper coordination of care and fulfillment of orders through completion.
  • The Care Manager demonstrates the ability to be self-directed and work effectively with others; strong organizational and documentation skills; effective written and verbal communications skills; critical thinking and problem solving skills; ability to resolve conflicts, including situations where there may be conflicting priorities. Responds to the needs of the patients and educates them how to resolve any issues.
  • The Care Manager promotes comprehensive care team model and becomes cross-trained as necessary to provide staffing support as needed. Exhibits effective communication to maintain excellent interpersonal relationships with the team.
  • The Care Manager assumes responsibility for developing and building upon the established bleeding disorders and thrombophilia program through community outreach, school liaisons, community based organization liaisons, committee membership, conference attendance and other duties.
 Requirements

  • Graduate from an accredited Registered Nurse (RN) Program
  • Bachelor of Science in Nursing (BSN)
  • Active California State Registered Nurse (RN) License
  • Minimum 2 years nursing experience in acute care or ambulatory setting
  • Basic Life Support (BLS)
  • HIPAA training
  • Ability to work onsite (not a remote role)
 Preferred

  • Experience in Case Management
  • Knowledge of CCS/GHPP/Medi-Cal regulations and requirements
  • Knowledge of third party payer methods and regulations
  • Experience in hematology/oncology nursing in an adult or pediatric setting
  • Bilingual English/Spanish

The Center for Comprehensive Care & Diagnosis of Inherited Blood Disorders (CIBD), a 501(c)3 non-profit organization, is committed to providing comprehensive care for patients and their families with rare, chronic inherited blood disorders. CIBD is a part of a nationwide network of over 140 Hemophilia Treatment Centers that use multidisciplinary teams of blood disorder specialists to provide expert comprehensive diagnosis, medical care, outreach, education, outcomes monitoring, surveillance, and prevention services to people with hemophilia, Sickle-Cell Anemia, Thalassemia, Von Willebrand Disease, and other related hematology conditions. CIBD is the regional grantee for the Western States Region/Region IX providing regional leadership, fiscal and administrative oversight, grants management, capacity building, clinical education, technical assistance, evaluation, and outreach to public and private health agencies at the local, state, and federal levels.
